DAY OF YOUr PROCEDURE

  • Please arrange to arrive at the Center 45 minutes before your scheduled procedure time.  
  • You MUST present valid identification and your insurance cards in order for the Center to perform your procedure according to Federal Trade Commission rules.  Valid identification could be:  driver’s license or photo identification.
  • Your co-payment, co-insurance or deductible will be collected on the day of your procedure, if applicable.  You should check with your insurance company as to the amount of your patient responsibility for a procedure at an out-patient facility.  If you have any questions concerning billing or payment policies, please contact our billing department at (518) 280-8460.
  • You will be given sedation for your procedure and will not be allowed to drive yourself home.  There is plenty of comfortable space for your driver to wait.  When you check in, your driver will need to check in with you.  A cab/bus driver cannot be your responsible adult we discharge you to. You will NOT be able to have your procedure if your driver does not check in with you.
  • When your procedure is complete, you will be taken to recovery for additional monitoring for 30 – 45 minutes.  Your family member/driver will be  encouraged.to be present when your physician discusses the findings of your procedure.  You will also receive written discharge instructions, any prescriptions and educational handouts at this time.  They will be reviewed with you and your family before discharge.

Important Insurance and Billing Information

  1. The Center participates with the following insurance plans:  
    1. Medicaid, Medicare, Railroad Medicare, Blue Shield, Community Blue, Empire Blue Cross, CDPHP, MVP, Cigna MVP, Empire Plan NYS, Unicare, Empire BC/BS, Healthy Living Partnership of Schenectady County,  UHC, Fidelis, Aetna, ASA Aetna Signature Administrators (must have ASA listed on the insurance card) Coresource, Fiserv Benefit Planners, Fiserv Harrington, Fiserv Harrington AVMA, Fiserv Health Employees, 1199 SEIU, Fiserv Wausau Benefit Service, Meridian Health, Principal Financial Group.
    2. If you are covered by a health insurance plan that is not listed above, we will be happy to submit a claim to your carrier as a courtesy to you.  However, you will be responsible for any balance remaining after your insurance has paid.  Please be sure that you have provided the correct mailing address and telephone number for your insurance carrier. 
    3. If you have any questions regarding this information, please call the Center and request to speak with the Billing Department.  Know that the Center will work with all patients to ensure payment is made.  If you need to set up a payment arrangement or want to seek information regarding our financial assistance program, the billing department will also assist with these requests.
  2. As part of your procedure in our Center, you may also receive bills for services rendered from the following in addition to the facility fee from our Center:

  • Saratoga Schenectady Gastroenterology Associates, P.C.
  • Saratoga Hospital Pathology Department Saratoga Springs Pathology, P.C.

As required by law, we make the following disclosures to you in advance of your procedure, along with our patient rights and patient responsibilities:

  1. Advanced Directives:  
    It is the policy of the Center that DNR orders be suspended while you are a patient at the Center. In the unlikely event of a life-threatening emergency, professional staff will initiate resuscitation or other stabilizing measures and transfer you to an acute care hospital for further evaluation, regardless of the contents of any advance directives, existing DNR orders or instructions from a health care agent. At the acute care hospital, further treatment or withdrawal of treatment measures already begun will be ordered in accordance with your wishes or advance directives. If a patient should provide his/her advance directive, a copy will be placed on the patient’s medical record and transferred with the patient should a hospital transfer be ordered by his/her physician. The Center provides information on advanced directives, such as: Health Care Proxy, Do-Not Resuscitate Orders, and Living Wills.  If you would like more information regarding advanced directives, please call 831-1550.  This will be reviewed again with you on the day of your procedure.
  2. Ownership Disclosure:
